Erectile dysfunction is a common problem that can severely impact the quality of life for both patient and his partner. Understanding and management of erectile dysfunction have evolved significantly over the last decades. Recent advances and breakthroughs in our knowledge of the vascular mechanisms responsible for the physiology of penile erection have led to the critical recognition of erectile dysfunction as a premature manifestation of cardiovascular disease. Screening for major health concerns, such as atherosclerosis, hyperlipidemia, hypertension, diabetes mellitus and other metabolic diseases has become an integral part of the evaluation of erectile dysfunction. The estimated incidence and prevalence of ED worldwide has not been clearly defined and varies greatly with country and age. However, all age groups of men seem to be affected. Despite persisting taboos surrounding ED, more men are seeking professional help and treatment. The advent of novel treatment options in recent years, most notably oral PDE-5 inhibitors, has shined a new light on the primary vascular cause and treatment target in some cases. Several new oral medications for ED have been introduced and others are emerging on the horizon.
